Whānau Health Social Worker
A whānau focused service to identify and support the whānau to address any social barriers affecting their health relating to housing, education, employment and cultural wellbeing.
We aim to:
Provide this service to any whānau
already accessing a Te Taiwhenua o Heretaunga Hauora Service
Help whānau identify their needs and
goals and support then in developing a plan to achieve
these
Work holistically with the whānau by
empowering and encouraging them
Feedback from this programme includes:
